Chronic Retainer For Care (CRFC)

Consider having a MyHomeAdvisor available to you 24 hours a day, 7 days a week and 365 days a year for a base rate of $1.65 to $3.29 per day depending upon your age, prescription maintenance needs and medical complexity... 

The Chronic Retainer For Care (CRFC) is paid based upon the private members age and health conditions. They are allotted a certain number of predetermined encounters per contractual year. Paid as arranged, the contract is valid for 1 year and automatically renews. 

At the conclusion of the annual CRFC contract, any remaining predetermined encounters will not carry over into the next contract, nor will any portion of the CRFC be refunded.

Private Contract Members enrolled in the My Home Advisor, LLC CRFC program will have their annual ACS Private Membership renewal fee of $25.00 waived for each year they remain in the CRFC Program.

Acute Fee For Care (AFFC)

MyHomeAdvisor will triage any requests for acute evaluation. If you are experiencing any changes in mentation, complaints of lightheadedness, stroke symptoms, shortness of breath, chest pain and or severe abdominal pains you will need to call 9-1-1 or seek medical attention through the Emergency Room. 

If you are in need of acute consultation, you will be paying an “acute fee for care” to schedule an acute consultation via phone, face-time or Concierge Service to your home. This consultation may result in additional fee’s based upon the complexity of the encounter payable at the time of your encounter to include, but not limited to lab orders, imaging or advanced study orders, specialty referrals and medication prescribing. 

MyHomeAdvisor does not accept your insurance as payment but CAN provide you with an itemized statement (via email or other means, at your request) that you may submit to your insurance carrier for your personal reimbursement for your Acute Fee For Care. 

By clicking on ‘terms of service,” you are agreeing to pay for your initial acute fee for care scheduling cost (per preference) and any additional secondary charges which may result from your consultation encounter such as procedure, medication prescriptions, labs and or imaging orders, etc as discussed. Your credit/debit card will be run before performing any procedure, prescribing any medications, or placing any lab and or imaging orders.

MyHomeAdvisor recognizes that life happens in stages and that with each stage of life, there are new priorities. 

CHILDREN:
Well-child checkups are regular visits for you, the primary stakeholder in your child’s health, development and well-being, to stay up to date on their physical, emotional and social milestones. Your child or teens regular checkups are an important part of making sure they stay healthy and on track to meeting their wellness goals. Well-child visits are about more than just your child’s physical health. These checkups are a chance to discuss health concerns, get support for your child’s well-being, and ask any questions. 

All children should have regular wellness visits and health screenings to ensure better health outcomes. ACS understands that the traditional “office visit” costs you both time and money by taking you away from your ability to work and by taking them out of their learning environments. 

MyHomeAdvisor offers a convenient concierge service at your home during the day, early evening, and limited weekends for children ages 2 through 18. Some of the things you and your MyHomeAdvisor may talk about at the home visit may be your child’s growth and development; family’s health; everyday life and challenges; and or their successes and milestones. 

During the home visit, your child or teen will have a complete physical exam checking for behavioral health, blood lead screening, developmental screening, eating or sleeping problems, family health history, general health and well-being, hearing or vision screening, how is your child growing and their oral health. 

ADULTS:
We understand that for some adults, there are not enough hours in the day to accomplish all the things that they need to for work, and family. Often, this comes as a sacrifice to their own healthcare needs. 

There are many people who make too much to qualify for healthcare plans. It is those people who cannot afford the healthcare they are offered. Although healthcare insurance is no longer required, a lack of healthcare insurance comes with risks of its own. 

ACS private membership does not offer healthcare insurance. The MyHomeAdvisor serve the ACS Private Member by filling the gap between prevention and unforeseen necessity by offering a variety of Adult Fee For Physicals.

MyHomeAdvisor may also conduct on site:
1. General purpose physical examinations.
2. Comprehensive Health Risk Assessments
3. DOT Certified Medical Exams 
4. Insurance specific screenings
5. Fit For Duty examinations
6. Well Child Checks
7. Sports Physicals 

NOTE: If your child is mRNA vaccinated, current guidelines for Sports Physicals require 12-lead ECG screening and blood lab work if the cardiac screening is abnormal. This would result in a Pediatric Cardiology referral in order to be cleared for sports activities. As such, MyHomeAdvisor will perform 12 lead ECG screening assessments on all ages who were mRNA vaccinated as courtesy if requested).

MyHomeAdvisor also recognizes that neither the government or non-government healthcare system can impose treatment on an individual for whatever reason and that a person has medical autonomy over their own bodies. Your MyHomeAdvisor is an advocate for your patient rights to make an informed decision based upon the delivery of complete, factual, and transparent information for you to weigh and balance when choosing for yourself. MyHomeAdvisor will assist you in completing any specific or neccessary administrative forms.

What we are not and will not do.

MyHomeAdvisor Private Party Programs are not a "work-around" to the current healthcare systems checks and balances regarding controlled substances. As such, our MyHomeAdvisor will not be prescribing chronic opiates/opioids, benzodiazipines and or certain muscle relaxers with abuse potential.

MyHomeAdvisor Private Party Programs are not health insurance and we are not a Christian "colaboration". We do not claim financial responsibility for the health of any person or for the diagnosis, treatment or resolution of any symptom or condition.

Because of the one-on-one in-home concierge services provided, MyHomeAdvisor will not be performing pelvic, rectal and or genitalia exams on either sex. In regards to womens health exam, referrals will be placed for GYN exams. Some non-invasive sexually transmitted infection (STI) screenings may be considered via external lab orders, but will not be conducted on site by the MyHomeAdvisor.

MyHomeAdvisor can evaluate and triage Acute Fee For Care infants, but will not accept responsibility for well child exams in children <2 years of age. This is because of the special programs neccessary to calculate growth curve and need for access to scheduled immunizations, a service not provided by MyHomeAdvisor.
